One of those ominous things that you didn't mention is phasing out
real health insurance and HSAs over 5 years.  That forces the middle
class to have HMOs.  IMO, HMOs were the foot in door for socialized
medicince.  They essentially *are* socialized medicine at the corporate
level.  Bureaucrats decide what treatments you will get, and how much
of your (actual or potential) salary pays for them.  Obamacare provides
a federal monopoly on HMO care (at least you could change companies with
a corporate HMO), and takes away the option of real insurance.

What?  You can still get private insurance under Obamacare?  Sure, but 
consider what happened with goverment education.  Thanks to the price
inflation of public schools, only the upper class can afford private
schools.  The Obamas don't send their kids to public school, and you
can safely bet they won't use the "public option" HMO either.
